These are meant to augment that information we send out to our callers. There is a generic Pre-Admission Assesment used to help reserve a treatment bed prior to the intervention. Also included is a payment authorization form for the intervention and escort (accompanied travel) to the treatment center, as well as a Hold Harmless agreement required for our interventionists. |
